Prescription Assistance Program
Wesley Nurse Health Ministries™/ Methodist Healthcare Ministries

• The Rx Assistance Program assists people who are unable to afford their prescription medication. 

Criteria:

• We cannot accept applications if you have any prescription drug coverage.
• We can only apply for medications that are brand name, not generic; and are listed by the drug companies
   as being on their assistance programs.
• Narcotics, such as pain medication, are not available through this program.

Participants must bring the the following items when applying:

• Medication bottles or prescriptions
• Proof of income (most current)
• Social Security card
• Driver's License
• Medicaid denial Letter
• Medicare supplement card
• Optional: Pharmacy printout (for medication costs)

All applications and services are confidential.
